I certify that my pet is current on its vaccinations for Bordatella, Rabies, and Distemper, and is currently on a flea prevention program.  If my dog is too young to have been given all of these vaccinations, they will be administered at the appropriate age.  I accept full responsibility for the acts of myself, my dog, and my guests while attending classes.  I understand that Teacher’s Pet Training Academy, LLC cannot be held responsible for any injury caused by my pet, or to my pet, and release Teacher’s Pet Training Academy, LLC from all claims of liability.  I certify to the best of my knowledge all the above information is accurate.

PAYMENT:  Payment must be received to reserve your spot in class.  All fees are non-refundable.  Once you press submit at the bottom of this form, you will be given the option to pay online (a small surcharge is applied) using PayPal, a highly trusted and secure merchant service provider.  If you wish to mail in payment instead (cash or check, please print a copy of this form and mail with payment to reserve your spot in class).  Fees are $90 for Puppy or Beginner Class, and $15 per elective.
